Where can i go to print off the questionares needed to file. I just took a CC class yesterday and want to get the ball rolling.

Print ONE copy of the questionnaire.  Fill out the top 1/3 (YOUR info).
Then make 6 copies.  I know you only need 5, but you will have 2
extra IF someone one screws one up.

Reminder: Fill out the DECCW application and all associated paperwork, but you must sign forms(do not pre-sign) in front of the people at the Prothonatary's Office.

Usually the clerk at the Prothonatary Office will check over your paper work and ask you to sign it in front of her. If you fill out your paperwork and sign it and get someone at work or somewhere else to notarize it and then take it to the Prothonatary Office and they check it and find a problem. You will need to start all over with new forms. What you had notarized will be null and void.
Lots safer, quicker and cheaper to just let the people at the Prothonatary Office do their job.

is it a good idea that anyone who sign's the questions that the answer for the answer's of 2, 3 ,and 4 be no? i'm thinking if they gave reason of why they think i wanted a permit, that might give the state a reason to deny me

I've got 7 so far, they all answered No the the questios that required a box being check, that's what i want, right?

For question 4, i explained to all my references why I was requesting the permit, and told them to answer yes, with subsequent explanation. I felt that it was the best option. Didn't have a problem.

4: Are you aware of the reason why this applicant has applied for a license to carry a concealed deadly weapon? Yes No

I told my references to answer honestly; some knew my reason (personal protection), and some did not.  Of course I can't recommend you follow my lead as I'm still waiting.
