Guide ad - In person or via phone

I take it I need to do this in person or can I actually call them to place my ad?

Nope, you can just give them a call... They don't need to see you in person... They can mail you the affidavit after it runs.

that's correct, they've done tons of these, they know exactly what you need...just call them.
