CCDW application questionnaire change

I tried to send the admin a message regarding this page but gave up trying to get through the "human verification" challenges.

Delaware has changed question #4, they eliminated the yes-no box and now require an answer.
The form was updated in April of this year. Note, previous links to the old form point to the new form.

To add, I recently submitted my application and the clerk was not satisfied enough that I had an original signature.
One signatures was original when scanned by the signer and sent as a PDF file to me. The second just 'looked like' it was not original.

She was pleasant about it and just assured me it was make it go through the process with no technical drawbacks.

Because I was going to re-do 2 of the references she also suggested I now use the new form without the yes-no check box.
I sent my buddies 2 copies of the completed forms and asked them to sign in blue ink and then submitted two originals.

So your references write "for personal protection and protection of property or both ".
